a. Duly Accomplished CECO Application Form (Annex A)
b. Endorsement Letter from the head of the Type 1 DE
c. Certified True Copy of Academic Credentials
d. Sworn Statement Annex B
e. Proof of payment of One Thousand Pesos (PHP 1,000.00) for the Application Fee for Certification
